Birth & Death Certificates

Computer down link allows the Shelby County Health Department access to vital records at the Missouri Department of Health in Jefferson City, Missouri.

Certified copies of Birth Certificates can be printed at our office while you wait. Certificates are available for any birth occurring in the State of Missouri after 1920.
Certified copies of Death Certificates can be printed for any person whose date of death is after 1980.

Birth and Death Certificate Application

• Birth Certificates are $15.00 each.
• Death Certificates are $14.00 for the first one, and $11.00 for each additional copy.

The Health Department does not accept credit cards.

A mailed application must be Notarized by a Notary before being sent. Please send Cash or Check with your application to:
