Partnering with Shelby County Health Department to provide services for Shelby County Residents

We would like the residents of Shelby Countyto know about our Home Health services that we offer and what will be required in order to qualify for them.

Lewis County Home Health provides services in Shelby County. We will be partnering with Shelby County Health Departments employees along with their contracted staff.

The choice is always yours. When the provider orders home health, you should be given a choice of the agencies available in your area.

Home Health Services Include:
Physical Therapy
Skilled Nursing performed by Registered Nurses
Home Health Aides for personal/hygiene care

  1. A medical necessity/skilled reason is necessary to receive home health services. Significant change in condition, new/changes in medication, new diagnosis, significant increase In weakness/falls, wounds care, catheter care, or recent surgery.
  2. To Qualify for Home Health please remember youmust be homebound. Patients mayleave the home infrequently for such things as medical appointments, grocery shopping, church and hair appointments. Health Insurances, including Medicare, believe that if you are able to leave your home without a taxing effort, then you are not home bound. We are Medicare/Medicaid certified State Licensed agency. We provide home health to homebound individuals who meet Medicare, Medicaid, VA or private insurance qualifications in Lewis and Shelby County.
  3. Home health must be ordered by a physician, physician’s assistant, or nurse practitioner. Home health collaborates with physicians throughout entire episode of care.
